The dealer form is a separate thing from the map: shoppers use the map, prospective dealers use the form. It usually belongs on its own page, linked from your footer or a "become a stockist" page.
Method 1: The app block
There is no app block for the dealer form yet, so skip ahead to the embed code below.
Method 2: The embed code
The Installation card on the same page gives you a code to paste. Copy it, then either:
As a page: Online Store → Pages → Add page, switch to the HTML editor, paste, save and publish.
As a section: Online Store → Themes → Customize → Add section → Custom Liquid, paste, save.
Where dealers land afterwards
By default a dealer stays on the page and sees a confirmation.
What to do next
Nothing happens when a dealer submits? Switch on the notification email, under Notifications on the same page.
Once submissions arrive, "Handling new dealer submissions" is what to do with them.
